In the Section entitled Vehicle/Trailer Info:
1. Record the Year of the unclassified / unregistered motor vehicle, trailer, or the vehicle upon which a special body is mounted.
2. Record the Make of the unclassified / unregistered motor vehicle, trailer, or the vehicle upon which a special body is mounted.
3. Record the Model of the unclassified / unregistered motor vehicle, trailer, or the vehicle upon which a special body is mounted.
4. Record the Vehicle I.D. number (VIN) of the unclassified / unregistered motor vehicle, trailer, or the vehicle upon which a special body is mounted.

In the Section entitled Add. Trailer Info:
5. Record the Length of a trailer in feet.
6. Record the type of trailer(ex. Utility, enclosed, etc.)

In the Section entitled Special Body Info:
7. Record the year of acquisition of the special body(It may be the same as the year provided for the vehicle upon which the body is mounted or it may be a different year)
8. Record the description of the special body and capacity if there is a capacity rating (ex. Cement mixer, 12 yard; Dump body, 8 yard; Bucket lift, 20' lift; etc.)
9. Record the acquisition cost of the special body only.
10. Indicate if the special body was acquired new or used.
